As the parent of One Eagle Scout, One Tenderfoot Boy Scout and one Webleo Cub Scout I finally got tired of filling in the B.S.A. Personal Medical Record Form. Seemed like every time we went on a campout in the outdoors ,canoe trip on white water, backpacking at a State Park I was filling out either a Class 1 , Class2 Form for Boy Scout Summer Camp, or the High Adventure Boyscout Jamboree Class 3 Form .If your Eagle Scout is going to Philmont this is the form he will need. I created this website to make being a Boy Scout parent easier. Here you can fill in your Boy Scouts details and then either save the information to your local computer or email. Personal Health and Medical Record does not store the information in any database. You fill out your Cub Scout or By Scouts information and save it, print it or even email it. Easy. Now if I can only find a way to make building a fast Pinewood Derby Car I will be set. Enjoy! There are three forms required by the Boy Scouts of America. Class 1 is the most common. Class 1 (update annually for all participants). Activity: Day camp, overnight hike, or other programs not exceeding 72 hours, with level of activity similar to that of home or school. Medical care is readily available. Current personal health and medical summary (history) is attested by parents to be accurate. This form is filled out by all participants and is on file for easy reference.
