Boy Scouts Class 1 Personal Health and Medical Record Cub Scouts

If you are new to the Cub Scouts or The Boy Scouts you soon find out that the BSA requires paper work. Whether your Cub Scout is a Tiger,Wolf ,Bear or Webleo and you are going Camping you will need to give a Class 1 Personal Health Form to your Den Leader. Same goes for Tenderfoot. Second Class, First Class, Star,Life or Eagle Boy Scouts . If your troop is going an a official BSA sponsored outing less than 72 hours long then you will need this Boy Scout Medical Record Form. To use this form simply fill in the blanks and save it your local computer or email it to the Den Leader or Troop Leader. We do not save your BSA information..
