Boy Scouts Class 3 Personal Health and Medical Record FORM NSJ-34412-01

Class 3  is the boy scout form required for Summer Camp and High Adventure. The BSA approved form is a bit more detailed and requires a doctors visit. As a boy scout camp volunteer I am required to have this form  filled out before going on the campout. The Boy Scouts require this form so the scouter will be physically fit for campout or high adventure trip he is going on. Philmont and Scout Jamboress use this form
